Continuity Patients
Throughout the three years of residency, each resident will develop, and expand, his/her own panel of patients. Many of these patients originally come from "No Doc" patients that the resident picks up while working on the inpatient medicine service. These will often lead to families joining the resident's practice. Additionally, residents may add new patients to their panels by working in some of our satellite clinics. Commonly, a resident will have 1-4 OB patients in the panel, 1-3 nursing home patients and occasionally a hospitalized patient or two.
When a panel patient is admitted to the hospital, the resident may elect to manage this patient on his/her own. Clinic faculty are available to precept the resident with these patien Often, panel patients are co-managed with the inpatient medicine team; especially if they have interesting illnesses or treatment needs. The resident will also have patients who end up in nursing homes on a temporary or permanent basis. These patients will be followed in these settings with a frequency dictated by the acuity of the patient's illnesses. Many residents make occasional home visits to homebound or high acuity patients.
