ABOUT US

Our office is a family practice clinic, which also serves as a family medicine teaching center. Resident physicians provide patient care under the supervision of Oregon Health & Science University faculty physicians. We offer a patient-centered medical home model of care and interprofessional teams of care providers, including physicians, nurse practitioners, physician assistants, nurse care managers, pharmacists, behavioral health providers, and medical assistants. And YOU, as the patient and family, are at the center of the team.

Cascades East Family Medicine Center is committed to serving the community and its health needs. No one will be denied access to services due to the inability to pay.

  • There is a discounted fee schedule available to patients who qualify. Please contact the Sky Lakes Patient Financial Assistance.

  • We accept Medicare and Medicaid.

MEDICATION REFILL POLICY

We encourage our patients to arrive 15 minutes prior to the start of their appointment and bring all of their medications. Patients should contact their pharmacy for refill requests. The pharmacy will contact Cascades East Family Medicine Center if there are no refills remaining.

PHONE CALLS

If you need to reach your doctor, simply call the clinic number (541) 274-6733. The front desk will forward a message to your health care team who will respond to your call within 48 hours. It is helpful to include alternate telephone numbers and best times to reach you as part of your message.

If you need medical attention after clinic hours, simply call the regular clinic line and give the operator your name, telephone number and nature of your illness. The on call doctor will return your call as soon as possible. Prescriptions will not be filled on weekends or after regular clinic hours on weekdays.

EPIC MYCHART

Cascades East encourages our patients to sign up for MyChart, which is a secure patient portal.  Using MyChart, patients can message their health care team with questions, view their medical record and manage their account. To register for MyChart just ask the front desk staff when you are checking into or out of your appointment.  

PATIENT ADVISORY COUNCIL

This council is made up of community members and providers and meets quarterly.  Cascades East is proud to partner with our Patient and Family advisory council to improve the patient experience.  If interested please inquire at the front desk.
